Keto Diet Offers Health Benefits Beyond Weight Loss

A new study challenges the idea that the keto diet is solely for weight loss. Researchers at Washington University School of Medicine in St. Louis found it offers significant metabolic benefits for people with obesity, particularly in liver and glucose metabolism. This discovery could reshape dietary recommendations for individuals with obesity and related metabolic conditions.
Beyond Weight Loss: Keto’s Metabolic Edge
The study compared the keto diet, the Mediterranean diet, and a low-fat, plant-based diet in 42 participants with obesity, prediabetes, and fatty liver disease. Participants were randomly assigned to one of the three diets, and all groups achieved a 10% weight loss and improved insulin sensitivity. However, the keto diet demonstrated superior outcomes in specific metabolic markers. Notably, half of the keto participants no longer met prediabetes criteria, compared to 29% on the Mediterranean diet and 7% on the low-fat diet. Liver fat decreased by 67% in the keto group, versus 45% in the other groups. These findings highlight the keto diet’s unique ability to address metabolic issues beyond weight reduction.
Surprising Findings: Fat Intake and Blood Levels
Despite consuming the highest amount of fat, participants in the keto group did not experience raised blood fat or cholesterol levels. This counterintuitive result was one of the study’s most striking observations. Additionally, the keto group showed the highest increase in glucagon, a hormone that aids in liver fat loss, and the greatest reduction in insulin levels. These hormonal changes contribute to the diet’s effectiveness in improving liver health and metabolic function. The study, published in Cell Metabolism, shows the metabolic advantages of reducing carbohydrate intake, even in a high-fat dietary context.

Dietary Approaches: A Closer Look
The keto diet, also known as the Atkins diet, derives approximately 70-80% of its calories from fat, with a significant restriction in carbohydrates, often limited to 20-50 grams per day. This macronutrient distribution forces the body into a state of ketosis, where it burns fat for energy instead of glucose. The Mediterranean diet, in contrast, focuses on plant-based foods, fish, and healthy fats like olive oil, with moderate carbohydrate intake and minimal red meat. The low-fat, plant-forward diet can derive up to 70% of its calories from carbohydrates, emphasizing whole grains, legumes, and vegetables while minimizing fat intake.
While all three diets effectively promoted weight loss, the keto diet’s impact on liver metabolism and prediabetes reversal was particularly notable. Samuel Klein, the study’s corresponding author and a professor of nutritional science, emphasized that reducing carbohydrate intake appears to offer the most significant metabolic benefits beyond weight loss. This finding could lead to a reevaluation of dietary guidelines for individuals with obesity, prediabetes, and fatty liver disease. Max Petersen, the study’s first author, highlighted the unexpected absence of increased blood fat or cholesterol levels in the keto group, despite their high fat consumption, as a key takeaway from the research.
